Figure 3A. Axial T1-weighted in-phase image. Normal-appearing liver.
Figure 3B. Axial T1-weighted out-of-phase image. There is loss of signal throughout most of the liver.
There are several hyperintense areas in segments III, IVB, V, and VI of the liver.
Diagnosis: Focal fat sparing.
Physics Discussion:
The loss of signal in the liver again signifies hepatic steatosis. The hyperintense “lesions” in the liver are not true lesions, but rather are areas of normal liver that appear bright in comparison with the signal loss in the surrounding liver. One should therefore not mistake these pseudo-lesions for true hepatic masses. Both focal fat infiltration and focal fat sparing demonstrate the same location predilection and are often identified in the region of the gallbladder fossa, adjacent to the teres ligament, anterior to the right portal vein, and in the subcapsular parenchyma.
Notes:
1. hepatic steatosis 肝脂肪变性
2. infiltration /ˌinfil'treiʃən/ /ˌɪnfɪlˈtreʃən/ n. 渗透
3. predilection /ˌpredlˈekʃən/ /ˌprɛdl'ɛkʃən/ n. 偏爱,偏好,嗜好
4. teres ligament 圆韧带
5. subcapsular /ˌsʌb'kæpsjulə/ /ˌsʌb'kæpsjulə/ 包膜下的
