Figure 8A. Axial STIR image of the pelvis; TI = 110 msec, performed on a 1.5T strength magnet. Diffuse high signal of the left iliacus muscle is seen. Linear high signal in the left sacral ala is also demonstrated. Note the intermediate signal intensity arising from subcutaneous and intra-abdominal fat.

Figure 8B. Axial STIR image of the pelvis; TI = 150 msec. There is increased contrast between the linear signal abnormality of the left sacral ala and the background of fat. Diffuse edema in the left iliacus is again seen. The signal arising from fat is decreased, and there is increased contrast between the left sacral lesion and normal bone.

Diagnosis: Left sacral fracture and left iliacus muscle strain.

Discussion

This case shows the effect of varying inversion times on image contrast. With the inversion time set at 110 msec, the signal intensity arising from fat is mildly suppressed but is sufficient to easily see the insufficiency fracture of the left hemisacrum and the edema in the left iliacus muscle. With the inversion time set closer to the inversion time of fat at 150 msec, there is increased signal suppression from fat and increased contrast between bone and fracture. However, the drawback of using a longer inversion time is a slightly increased imaging time.
