256.Coronary artery aneurysms associated with SLE

Aneurysms of the coronary arteries are characterized by local saccular or fusiform dilatation that exceeds the normal vascular diameter by at least 1.5 times. The incidence of coronary aneurysms in patients undergoing coronary angiography ranges from 0.3 to 4.9%. There are no clinical findings which are specifically suggestive of coronary artery aneurysms. The most common cause of coronary artery aneurysms in the U.S. is atherosclerosis; congenital malformations ranks second; and Kawasaki disease (mucotaneous lymph node syndrome) is the best studied cause. Other less common causes are infection, syphilis, trauma, septic emboli, polyarteritis nodosa, SLE, Ehlers-Danlos syndrome, scleroderma, Marfan's and Takayasu's arteritis. The most common sites of coronary aneurysm are the right coronary artery, followed by the left anterior descending and left circumflex arteries. Patients with atherosclerotic aneurysms typically have multiple aneurysms and are likely to have stenotic and obstructive lesions as well.
冠状动脉瘤的分类依据其呈囊状或纺锤样扩张,其扩的管经超过正常血管直径约1.5倍。行冠状动脉血管成像的患者发现动脉瘤的概率大约在0.3至4.9%。冠状动脉瘤缺乏特异性的临床表现。美国地区最常见的冠状动脉瘤起因为动脉粥样硬化,其次为先天性异常,研究最深入的为川崎病(皮肤粘膜淋 结综合症)。其余较少见的原因为感染、梅毒、创伤、脓毒性栓子、多发性动脉炎、系统性红斑狼疮、 Ehlers-Danlos 综合征、硬皮病、马凡氏症候群伴多发性大动脉炎。冠状动脉瘤最好发于右冠动脉,其次为左前降支和左回旋支。患有动脉粥样硬化性动脉瘤的患者一般为多发。同时件有局灶性及阻塞。
