Diagnosis: Herpes encephalitis.
Discussion:Case 11 is a case of a young patient with altered mental status; the possible right insular signal abnormality on the T2W image is confirmed on FLAIR as the suppression of CSF results in improved contrast between normal and abnormal
parenchyma. The FLAIR image also revealed a subtle abnormality in the contralateral insula, which was undetectable on the T2W image. The finding of bilateral, asymmetric parenchymal abnormalities in this patient with altered mental status raised the possibility of herpes encephalitis, which was confirmed on subsequent CSF studies.
