A 66-year-old man undergoing routine follow-up imaging after an orthotopic liver transplant. There are coarse calcifications of the bilateral lungs with peripheral calcifications of the spleen. This is a case of metastatic pulmonary calcifications secondary to orthotopic liver transplantation.

Metastatic pulmonary calcification occurs when calcium is deposited in normal lung parenchyma, primarily as a result of metabolic derangements in which there are exceedingly high levels of serum calcium and phosphate. The most common cause is chronic renal failure, followed by hyperparathyroidism (both primary and secondary), sarcoidosis, vitamin D intoxication, destructive osseous lesions, and IV calcium therapy.

Another more rare but well-documented cause is orthotopic liver transplant. Patients who develop these calcifications following liver transplant are generally asymptomatic, and these lesions are incidentally discovered following unrelated imaging.
