274.Burst fracture of C5（2）

Diagnosis:  Burst fracture of C5 with a flexion teardrop-type of mechanism.  Cord contusion with intramedullary hemorrhage, small epidural hematoma, and small herniated nucleus pulposus at C3-4 and C5-6.

The burst fracture seen here involving C5 also has characteristics of a flexion teardrop fracture. Burst fractures are produced by a combination of flexion and axial loading while flexion teardrop fractures involve such severe flexion that the anterior corner (teardrop) is separated from the posterior half of the body, which is displaced into the spinal canal, damaging the cord.  The MR shows clear evidence of this type of mechanism in the disruption of the anterior longitudinal ligament and partial disruption of the posterior longitudinal ligament.

诊断:C5爆裂性骨折伴屈曲泪滴型机制。脊髓挫伤伴髓内出血，硬膜外小血肿，C3-4、C5-6处髓核小疝。图中C5所示的爆裂骨折也具有屈曲泪滴骨折的特点。爆裂性骨折是屈曲和轴向载荷共同作用的结果，而屈曲泪滴骨折屈曲严重，导致前角(泪滴)与泪滴体后半部分分离，而后半部分移位进入椎管，损伤脊髓。MR明确显示了这种机制在前纵韧带断裂和后纵韧带部分断裂中的作用。
