Tylenol Recalled After Found Laced with Cyanide

At least seven people died after taking Tylenol acetaminophen that was contaminated with cyanide.  The realization led to J&J to recall all Tylenol capsules and led to new anti tampering bottled to protect customers from third party attacks.

Why

significant

No one was ever arrested or convicted for the deaths in the Chicago area, but the concerns over product safety led to the largest recall in J&J history.  Eight bottles were ultimately discovered, 5 that led to deaths and three more in an investigation.  It is supposed the bottles were tampered with after delivery to retail outlets.
